FibroScan (transient elastography) measures how stiff the liver is — a direct read-out of scarring — and how much fat it contains (CAP score), in five painless minutes. It has replaced liver biopsy for most patients with fatty liver, hepatitis B/C and alcohol-related disease.
What the numbers mean
Stiffness in kilopascals (kPa): under ~7 is normal, 7–10 suggests significant fibrosis, above 12–14 suggests cirrhosis (thresholds vary by cause). CAP in dB/m grades fat: under 238 normal, 238–260 mild, 260–290 moderate, above 290 severe. Dr Paliwal interprets them in context — a high number is not a sentence.
Who should have one
Anyone with fatty liver on ultrasound, raised liver enzymes, diabetes or obesity, hepatitis B or C, regular alcohol use, or a family history of liver disease; and for monitoring every 6–12 months during treatment.
How it is done
You lie on your back; a probe is placed between the right ribs; ten painless pulses are recorded. Fast for 3 hours beforehand. Results are immediate and explained the same visit.
Limitations
Less accurate in severe obesity (an XL probe helps), acute hepatitis, congestion and after a meal. Dr Paliwal combines it with blood scores (FIB-4) for a complete picture.
